  • ONE of the following Column A Primary ID documents; OR
  • TWO of the following Column B Secondary Documents; OR
  • ONE of the following Column B Secondary Documents and ONE Column C Supporting Document
  • Below is a table the documents that are considered Primary, Secondary, and Supporting.

  • US Tax Form (W-2, Form 1099, 5498 or 1098)
  • Certificate of Citizenship (or Naturalization)
  • Medicare Card (A, J, H, M, T)
  • Court Order for Legal Name Change
  • New York City ID
  • Cedula ID Card
  • Utility Bill (Gas, Electric, Telephone, Cable, etc.)
  • Financial Record 
    Bank, 401(k), or Investment/Brokerage Statement
  • College/University Statement
  • Mortgage Statement
  • Life, Health, or Other Insurance Statements (Auto, Home, Boat, etc.)
  • Medical Records (Doctor, Hospital, or Clinical)
  • Pay Stub
  • Lease Agreement
  • Vehicle Registration Card
